Mission of Agency
We believe that all individuals have the potential and ability to learn and grow when provided with the necessary tools and confidence to do so.
Our outcome is to empower individuals with developmental disabilities to practice self-determination in their daily life. Our goal, leading us toward our outcome, is the practice and the support to others in the practice of allowing and promoting everyday life, natural experiences in the life of a person with a developmental disability. All activities of Community Opportunities shall be directed in accordance to the highest level of wellbeing of the individuals served. All agency efforts shall be conducted in a manner that promotes self-respect, enhances image and encourages a positive learning environment.
Person-Centered Planning
Specifically tailored around the goals, dreams, visions, and preferences of each individual, the Plan of Care (POC) for individuals serves as a guide for provision of each service selected by the SCL recipient and/or the guardian. The document includes specific information for the individual, including a profile of his or her personality. It also provides social history and health and safety concerns. Moreover, the POC explains in detail the outcomes that each individual identifies in a team meeting and the necessary steps to assist in achieving the goals.
